THE FOLLOWING ARE REQUIRED REGISTRATION DOCUMENTS PROVIDED BY PARENT/GUARDIAN:

Proof of your child's age: State issued birth certificate.
Parent Photo Identification: ID card such as Driver's license, state ID or passport (Driver's license cannot be used as proof of residency).
Proof of residency (Two (2) required): Acceptable documentation includes: lease, sales agreement, or mortgage statement plus one additional document-utilities bill, cable bill, etc. If residing with a district property owner, the property owner must sign a notarized Affidavit of Multiple Occupancy form in addition to providing additional documentation. All Affidavits are reviewed by the Residency Verification Officer.

Please Note: Cumberland Valley School District reserves the right to verify residency information for confirmation and factual accuracy. Falsifying documents is a misdemeanor of the Third degree in PA, punishable by a fine up to $2,500.00, imprisonment for one year or both. Additionally, you must subject yourself to a civil action for damages if it is later shown that the above child is not properly entitled to free school privileges.

Immunizations records: Acceptable documentation includes: the child's immunization record, a written statement from a former school district or medical office.
Consent for Release of Information form (found in online registration forms).
Discipline Sworn Statement (found in online registration forms).

RECOMMENDED DOCUMENTS:

Report cards/transcripts (in order for Middle and High school students to get a schedule, most recent report card, schedule or transcript is required)
Most recent physical and dental forms
Special education documents (IEP, ER, RR, NOREP) if applicable
Custody order if applicable
Any other records relevant to your child's education.
